It can be tricky, but here's an easy way you can do it. On the main Twitter page, look for a link on the left column, near the top. It will say something like 107 Photos and videos. That will take you to the media page. Then just right click on the photo you want to save and choose Save Image.

It must be protected. I saved the image on my cpu, but I still can't copy/paste. When I paste it, I get: CzFuDufUcAATPAi.jpg.

After you've saved it locally on your computer, you have to attach it to your new message/message reply. That will be under Additional Options.
It's a pain in the neck, but typical behavior of any web based app.
